Attitude to mistakes

Why does empowerment sometimes not work for either the individual or the organisation? How do we keep control and not slip into anarchy? How do we get people to take responsibility and bring energy and enthusiasm to their work? How can we encourage them to take the initiative and innovate? Tight control management gives us few mistakes but we get little in the way of involvement or innovation. A loose style of management increases involvement and innovation, but also the chances of errors and duplications. So which is best?
